GAME & COMPETITION SCHEDULES

Prior to participating in sports at Laguna Middle School, students must provide the Athletic Department with a current and up-to-date Sports Physical Form. The form can be accessed and submitted via the links below.

Participating students must also provide a current address, emergency contact, insurance, medical and health history information. This information will be entered into AERIES during the Data Confirmation process at the beginning of the year. If your student will be participating in Laguna Athletics at any point in the year, please make sure to complete this accurately.
